Heart of Kansas Family Health Care Additional Programs:

• Sliding Fee Schedule is based upon patient’s income and number of dependents.
• Farm Worker Vouchers are available for patients in crop harvesting/agricultural jobs.
• Early Detection Works (EDW) vouchers are available to female patients who qualify for Well-woman services.
• Vouchers for initial office visits are available for Physicians contracted with Heart of Kansas for specialty services including Cardiology, Ophthalmology, Dermatology, Nephrology, and Neurology.
